Details New financial information must be submitted every 6 months and a new application every 12 months. An advocate must act as the contact person on behalf of the patient. Program Requirements
Information Each application and all supporting documentation, including prescriptions, should have the patient's unique ID number (assigned by Bridges To Access). Proof of income requirements are very strict, and must be from the last 45 day time period.

Application Process
App Process After completing the application, an advocate must call the company and pre-enroll the patient. The application and supporting documentation must then be mailed in. Once eligibility is confirmed, the patient will receive up to a 90-day supply of the requested medication via Walgreen's mail-order pharmacy. Upon pre-enrollment by phone, a voucher is then activated. The voucher can be taken to any local pharmacy and will grant the patient a 60-day supply of the medication for a $10 copay. The patient must provide a prescription to accompany the voucher.

Eligibility
Eligibility Eligibility is based on 250% of the FPL and no coverage for the requested medication. Medicare eligible patients not enrolled in a Part D plan and who do not qualify for Social Security's Low Income Subsidy program for Part D are also eligible. An advocate must act as the contact person on behalf of the patient. Any healthcare worker involved in the patients care, such as a social worker or someone in the healthcare office, may serve as a healthcare advocate.
