Program Details
Details Up to a 90-day supply is shipped to the doctor�s office. For refills, patients can send in a new application, or the doctor's office can call and request a refill. A new application with documentation is needed once a year.

Application Process
App Process Anyone concerned can call and request an application. Applications are faxed out. Completed applications must be mailed back. Both the patient and doctor are notified in writing of acceptance or denial. Please allow up to 4 weeks for processing and delivery of medication.

Eligibility
Eligibility The patient can not have any private or public insurance and have an income at or below 200% of the Federal Poverty Level. Medicare Part D eligible patient's who did not enroll may still be eligible for this program. For patients who have insurance and a prescription to a medication on this program but cannot afford the co-pay, or are in extreme hardship, may still be elligible for assistance on this program. For these cases, a Hardship Form can be filled.
