| Details | Medications in the programs will cost either $20, $30, or $40 for a 90 day supply. The medication can be sent to either the patient's home or the doctor's office. Controlled substances cannot be mailed to P.O. Boxes or doctor's offices. The patient must contact the program and arrange payment in order to receive refills.
